Please print the Credit Application, fill in the information requested, and fax to: 812-981-4993. We suggest that you fill out the Credit Application now, before your first order, to prevent a delay. We will respond via email, fax, or U.S. Postal Service to provide you with a credit limit, an account number, and a password as soon as we are able to establish credit.
